I’m gonna admit, he’s a long way from my favorite character. His author went way too hard on tiny variations that made his two scenes a pain to code, and I don’t really care for his personality. That said, he’s something we don’t have any of in the game – cuntboys. So for all of you that enjoy that, there he is.

Well, a flat-chested tom-boy would be no breasts (like these cunt-boys) and act like a boy, more so than a girl.
Is the difference deeper than outward appearance then? Cunt-boys don’t have fallopian-tubes? No milk-glands?

You can distinguish 3 different gernder. The genetic, biological and personal gender.
TITS (and many SciFi and Fantasy series) ignore the genetic part (you can have alien hybrids). In reality, you have only XY men, XX women and some infertile mutants (e.g. XXY men).
Biological, men have a penis and testicles and women a vagina, womb and ovaries. (+ some other differences like big breasts or wide shoulders.)
Lots of testosterone = person looks like a man + male genitals.
Lots of estrogen = person looks like a woman + female genitals.
But in some cases a child is born with genitals between male and female.
Biological, Syri is a man with a female body and Gil would be a woman with a male body.
So, biological, cunt-boys and tom-boy are the same. (Provided you ignore the genetic part and the fertility.)
Note: Even human men can give milk. (Under appropriate conditions like self-hypothesis.)
The personal gender everyone must know for themselves. Syri makes typical masculine things, but sees herself as a woman.

That’s not remotely accurate because you aren’t considering circumstances. Syri is a female who, through an act of science, has a penis. Saying she’s a man even though she wasn’t born with that penis and didn’t grow it naturally, is wholly idiotic.
Its only the biological gender definition. If you have a penis and can (theoretically) impregnate someone, then you are a male.
If you can get pregnant (because you have a vagina and a womb), then you are a female.
If you can both, then you are a herm.
With the proper stimulation men who were born as men could produce milk for babies to drink, yes i mean breast milk

All right, let me try to sum it up for you.
There is more to having a masculine or feminine appearance than simply having a dick/pussy/tits. Consider: in the game, the masculinity or feminity of the PC’s face is a sliding scale, and the middle of the scale is “androgynous” facial features. That is one way in which gender is outwardly expressed through something other than sexual characteristics. There are many other aspects of physical appearance that can indicate gender, and no, we’re not talking about easily changeable details like style of hair or clothing.
“Tomboy” only describes females, mainly because it’s ridiculous to say a boy is a boy, that’s redundant. But it isn’t always about appearance. It can mean hobbies and other personal opinions, or it can be about behavior. A girl who likes rough-and-tumble sports like rugby or enjoys playing in the dirt might be called a tomboy.
Also, tomboys vary in how feminine or not they might appear. For most people described as a tomboy, they are clearly female. They may have no breasts or very small breasts, but they possess other generally feminine features such as their face. They may have short hair but it is often in a feminine style. Et cetera.
It’s erroneous to assume that we tell gender based on the presence of breasts or the type of genitalia a person is sporting. It goes far beyond that. And that’s the distinction between a “cuntboy” and a “flat-chested tomboy”. A cuntboy is NOT feminine at all in appearance EXCEPT having a pussy. A “dickgirl” is NOT masculine in appearance at all EXCEPT having a dick.
